Nondiscrimination Notice

BioVantra is committed to providing its clinical laboratory and diagnostic services without discrimination, consistent with applicable federal and state laws.

1. Commitment

BioVantra does not exclude people from, deny benefits to, or otherwise discriminate against individuals on the basis of race, color, national origin (including limited English proficiency), age, disability, sex (including pregnancy, sexual orientation, and gender identity), religion, or any other characteristic protected by applicable federal or state law.

2. Services and access

To the extent applicable to its services, BioVantra:

  • provides appropriate auxiliary aids and services, free of charge and in a timely manner, to people with disabilities to facilitate effective communication, such as qualified sign language interpreters and information in accessible electronic formats;
  • provides language assistance services, free of charge and in a timely manner, to people whose primary language is not English, such as qualified interpreters or information written in other languages;
  • maintains administrative practices intended to support equitable access to laboratory services in coordination with ordering clinicians and healthcare organizations.

3. How to request assistance or file a complaint

If you believe that BioVantra has failed to provide these services or has otherwise discriminated on a basis described above, you can file a grievance with:

You can file a grievance in person, by mail, by fax, or by email. If you need help filing a grievance, BioVantra is available to help you.

4. Federal complaint route (Section 1557 and related civil rights laws)

To the extent BioVantra is a covered entity under Section 1557 of the Affordable Care Act or other federal civil rights laws, you may also file a civil rights complaint with the U.S. Department of Health and Human Services, Office for Civil Rights, electronically through the Office for Civil Rights Complaint Portal at ocrportal.hhs.gov/ocr/portal/lobby.jsf, or by mail or phone at:
